Release of Liability
In consideration of my child's participation in the Vacation Bible School (VBS) organized by Troy International Church of God, I, the undersigned parent/legal guardian, hereby acknowledge, understand, and agree to the following:
Voluntary Participation: I understand that my child's participation in VBS is voluntary, and I assume all risks associated with their participation, including but not limited to physical injury, illness, or property damage.
Assumption of Risk: I understand that VBS activities may include, but are not limited to, games, crafts, outdoor play, Bible lessons, and snacks. I am aware that these activities may involve certain risks and hazards, and I freely and knowingly assume all such risks on behalf of my child.
Release and Waiver: I hereby release, waive, discharge, and covenant not to sue Troy International Church of God, its pastors, employees, volunteers, agents, and representatives (collectively referred to as "the Church") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury, or illness (including death) that may be sustained by my child while participating in VBS, whether caused by the negligence of the Church or otherwise.
Indemnification: I agree to indemnify and hold harmless the Church from any and all losses, liabilities, damages, costs, or expenses (including attorney's fees) that the Church may incur as a result of any claims, demands, actions, or causes of action brought by or on behalf of my child arising out of or related to their participation in VBS.
Medical Treatment: In the event of an injury or illness during VBS, I hereby authorize the Church staff to administer first aid and/or seek emergency medical treatment for my child if deemed necessary. I agree to be responsible for all costs associated with such medical treatment. I will make every effort to ensure that the emergency contact listed above can be reached in a timely manner.
Behavior Expectations: I understand that my child is expected to behave respectfully and follow the instructions of the VBS staff. The Church reserves the right to dismiss any child whose behavior is disruptive or poses a safety risk to themselves or others.
Entire Agreement: This Participation and Release of Liability Form constitutes the entire agreement between the parties and supersedes all prior or contemporaneous communications and proposals, whether oral or written.
Severability: If any provision of this form is held to be invalid or unenforceable, the remaining provisions shall continue to be valid and enforceable.
By registering for VBS, I acknowledge that I have read this Release of Liability Form, understand its contents, and voluntarily agree to its terms and conditions on behalf of my child.
